Why does the top of my left foot only swell up from time to time?

I have gone to the dr and the er several times for the sweeling of my foot. Noone has any idea why it swells up I can’t get a shoe on and its very painful. Blood test say nothing er asks me why I continue to show up since there is nothing they can do. Dr ruled out gout. I have had several iv’s with anti biotics and nothing works only the top of my foot sweels and so much I can’t walk. What causes this?
